Back in Hosp and Panicking
I’ve been admitted to hospital and diagnosed with Pelvic Inflammatory Disease. Was initially given intravenous Ceftriaxone and oral doxy. Have refused anything else until I’ve spoken to doc because I think they’re being far too liberal with the drugs without knowing what is causing the infection (it may be strep b, neither of these abx work for that. Clindamycin does but I cannot cannot cannot risk it). My gut started gurgling a lot yesterday. I’ve asked for zinplava but it’s rare on the NHS. I’m so ill I will have to agree to something eventually but thoughts welcome and if someone could link me to the list of most to least ‘safe’ I’d appreciate it. Can’t find it on search. Obvs am hitting the S Boulardii.
Contracted C Diff from a round of Flucloxacillin, November 2020.

Re: Back in Hosp and Panicking
Can you request to see a Infectious Disease specialist with your history and difficult with CDIFF???? Since you are in the hospital I am certain they have one. Request they work with your current doctors. Mine put me on Dificid while I was in the hospital on IV antibiotics. Most will put you on Vanco...but they can help also. I am allergic to Vanco now.
It made a difference, I did get CDIFF but when released the DIficid started working quickly and I was able to push through rather quickly and without all the problems I had in the past.
If they can't get you and ID, have your doctor consult one. So SORRY you are going through this, it is always so scary. But do know with your knowledge you can get through this. Sometimes the antibiotics are necessary We also know much more about treating CDIFF now.
Wishing you the best......keep us updated

Re: Back in Hosp and Panicking
Thanks for replying, Nanci. I don’t think they will let me speak to anyone like that but they did ask microbiology on my behalf if I could have Zinplava - who told them they didn’t think it was necessary! With my history! They don’t give preventative antibiotics in the NHS so I’d have to get Dificid privately, costs £1k and I wouldn’t know who could prescribe now I’m in hosp. They are even told in the guidelines not to recommend pro- or prebiotics to patients! I have some Dificid tabs in a drawer at home from 2021 but they’ve expired.
The doc said they can’t do anything else unless I take the abx and if I don’t I might get sepsis - but I keep wondering if this is even bacterial. You can get viral PID too, and candida-induced, though it’s rare.
This pm I had pain like in 2020 & gut is gurgling. Weirdly it’s three years last week since I was in this hosp with my first bout of C Diff. Only a few days ago I was looking at a cardigan from back then and wondering if I could bring myself to wear it. Shouldn’t have touched it, jinxed!

Is there any way they could do a culture to see what bacteria it is?

Re: Back in Hosp and Panicking
I would be asking if metro (flagyl) was suitable.
It's broad spectrum and also kills most strains of C.diff if prescribed for a minimum of 10 days.
Shorter or weaker courses are more likely to cause c.diff so remember to ask for the 10 day course as historically prescribed as c.diff treatment.

They have done a swab that showed strep but the consultant says that wouldn’t cause PID, even though I’ve found studies that say it does. They repeated but most times swabs come back negative. I’ve got a feeling it’s not bacterial but they said if I don’t take the abx they won’t consider a laparoscopy, which is what I really want. It’s a nightmare!

Really?! I actually thought of you once saying how metro is like letting a bomb off in the gut so I’ve refused to take it even though it’s part of PID protocol. I thought as it’s no longer used for c diff it was a bad idea. . . ? This is so awful, trying to decide.

It is not used for c diff anymore, but if it is part of the treatment protocol it is one of the antibiotics that is much less likely to cause it and may help protect you from getting it if you are on other antibiotics.

I see. Thank you, I’ll have to discuss with the consultant. I wish they would treat me for all other possible causes before going down the bacterial route!

Appreciate it, Nanci. The intense pain has lessened a little but over all I feel worse. Wondering if some of it is metro side effects . . . I heard them talking about adding penicillin to the protocol but that’s what triggered C Diff in 2020 so I won’t know what to do if they suggest it. I just can’t face getting it again. This is truly awful but that was even worse. Thanks for asking.

I am so sorry you are going through this....they really need to find out what is going on. I hope they have done all necessary test. A CT????
Metro made me VERY sick and it did last for days until it was out of my system.
Wishing you better days ahead!!!

Re: Back in Hosp and Panicking
Thank you. They did a CT, ultrasound & an MRI. Got adhesions, most likely from two years of untreated infection/inflammation (despite numerous trips to the hosp where they kept saying it was ‘functional pain’, an absolute non-diagnosis). They are following the general protocol for PID minus the penicillin because of the C Diff risk, but I don’t think doxy and metro cover the particular bacteria I have. My gut is so painful now, am typing this doubled up on the bed. I asked if I can stop since they’re not really helping and I feel over all worse but they said it’s best to finish the course. So I’ve taken two types of abx, neither of which fully cover the swab result, and because I’ve now re-damaged my microbes I don’t want to push for penicillin and risk tipping into C Diff (praying am not there already). Argh.

It may work out just fine, many have taken antibiotics and do not get it again........hang in there . Sending positive thoughts your way.....

Re: Back in Hosp and Panicking
Thank you, Nanci. D this morning, though. I’m stopping both the doxy & metro and upping the probiotics. No idea if this is the sensible option.
