Vancomycin Taper Question

Second round of Vanco
250- 4 times a day for two weeks
250-once a day for a week
250- every ither day for 8 days
250-every third day for 15 days

Has anyone seen this taper?? It seems like a big jump from 4 times a day to once a day!

I hope you are doing well. I hope you don't mind if I piggyback on your question. I'm looking for a similar answer but I would like to know if vancomycin should always be tapered. My RX has me on 125mg 4xday for 14 days and that is it. I would also love to hear your question answered so I can possibly discuss these things with my doctor. Thanks for letting me impose. I appreciate it.

There are many different tapers out there. The one I have recently been prescribed (But I am not taking yet bc I am waiting for my final results) is 125mg 4 times a day, then 125mg twice a day, then once a day and so forth. You are doing 250mg once a day, which is the same total milligrams as mine, mine is just broken up. Was it a specialist who prescribed it for you, or a PCP?

GI dr. I called and demanded a proper taper, I am seeing him next Wednesday.

Good for you . You can't just jump from 1 gram of vanco/day to 250 mg/day. The taper must be slowly reduced , and sometimes you can have surprises when you go down to one dose per day . some of us relapsed on that dosage. You can look up Dyalinn posts , she has a taper/pulse from Dr.Kelly and maybe you can print that and show it to your doctor.
